Think about Me
Good Care Guide is working with national partners and local authorities to help improve transparency in care across Gwent
Launched 2015.
Good Care Guide, in association with the Aneurin Bevan University Health Board and the South East Wales Academic Health Science Partnership, is delighted to present 'Think about Me'.
Why 'Think about Me' is important...
Quality and perceptions
The pilot programme, now live across Gwent, has been made possible with the help of funding from the Health Technology Challenge. It's designed to support the work of the Care Home Ask and Talk (C.H.A.aT.) service, where teams of volunteers go into homes to talk to older people and their families about their experiences with care. It will help improve the quality and perceptions of eldercare provision in Gwent, as well as increasing much-needed transparency across the sector.
Capturing conversations
Good Care Guide will be used as a platform to capture conversations, and extend reach of residents' experiences to other families across Gwent. It will give older people and their families more of a say about the care they receive, and highlight to care homes where they can make improvements, whilst at the same time letting them celebrate that which they already do well.
80 out of 96 homes across Gwent have joined the pilot.
We'd like to thank the following organisations for their ongoing support:
